DIEP Flap vs Implant Reconstruction

A DIEP (deep inferior epigastric perforator) flap rebuilds the breast with skin and fat from the lower abdomen while attempting to preserve the abdominal muscle; implant-based reconstruction uses a tissue expander and/or implant.

The choice weighs a longer own-tissue operation against a shorter implant-based one, with different donor-site, radiotherapy and long-term considerations. It is decided with the surgeon.

How the two options differ

 DIEP flapImplant-based
Tissue usedOwn abdominal skin and fat; muscle largely preservedTissue expander and/or breast implant
Operation & recoveryLonger microsurgical operation; longer initial recoveryShorter operation; may be staged over visits
Donor siteAbdominal scar; possible weakness, bulging or herniaNone
RadiotherapyMay be considered after radiotherapy; radiation can still affect tissueRadiation may raise hardening, distortion and failure risk
Long termNo implant; natural change over time; revision still possibleNot a lifetime device; future imaging, revision or exchange possible

Points in favour of DIEP flap

  • Uses the patient's own tissue with no permanent implant
  • Attempts to preserve the abdominal muscle (though donor-site risks remain)
  • May be considered after previous radiotherapy

Points in favour of implant-based

  • Shorter operation and no donor site
  • May be staged, and can be direct-to-implant in selected patients
  • Suitable when skin and blood supply are favourable

Limitations

Common questions

Is a DIEP flap better than an implant?
Neither is best for everyone. A DIEP flap avoids an implant but is a longer operation with a donor site; an implant is shorter but is not a lifetime device. Suitability is individual.
Does a DIEP flap avoid abdominal problems?
It attempts to preserve the abdominal muscle, but weakness, bulging, hernia and wound problems remain possible.
